What the research says
This is the strongest, most rigorous evidence cannabis has produced. Purified CBD (Epidiolex) was FDA-approved in 2018 for Dravet syndrome and Lennox-Gastaut syndrome after large placebo-controlled trials (the CARE trials) showed significant reductions in seizure frequency with an acceptable safety profile. It's a genuine, guideline-recognized antiseizure medication.
It's important to note the approved product is pharmaceutical-grade CBD — not the CBD oil sold in dispensaries, which varies wildly in dose and purity. The trials used measured doses of hundreds of milligrams of pure CBD, given on a schedule.
Minor cannabinoids are where the pipeline looks next. CBDV reached a Phase 2 add-on trial for focal seizures — it did not meet its primary endpoint, though it was well tolerated and some secondary measures showed signal. CBDVA inhibits T-type calcium channels, a mechanism relevant to seizure control, and CBCA showed anticonvulsant activity in a mouse model of Dravet syndrome. All of this is early or preclinical; none of it is a treatment today.
The honest caveats
This is the rare case where cannabis's evidence is genuinely 'Strong.' But it's about pharmaceutical CBD under medical supervision — not whole-flower, not THC (which can lower seizure threshold), and not self-dosing. For anyone with seizures, this is exclusively a clinician-guided conversation. CBD also interacts with common seizure drugs (especially clobazam), so doses must be managed carefully.
