Arthritis & Joint Pain

The two largest, most rigorous trials found no benefit over placebo. Early small trials suggested promise, but the signal has not held up.

What the research says

The honest headline: the best-designed osteoarthritis trials have found no benefit. In the largest placebo-controlled trial to date — a 2023 Vienna study of 86 people with knee osteoarthritis given 600 mg/day of oral CBD alongside their usual pain medicine for 8 weeks — CBD produced no benefit over placebo on pain or physical function. A 2025 trial (the CANOA study) in knee osteoarthritis using a CBD-rich oil for 60 days likewise found no significant difference from placebo. Earlier small trials had hinted at benefit, but as trial quality has improved, the signal has faded.

Because inflammation drives arthritis symptoms, anti-inflammatory compounds (beta-caryophyllene, CBD) are the ones most often discussed. But 'often discussed' is not the same as 'proven.' The largest, highest-quality trials so far point the other way: no benefit over placebo.

The honest caveats

Most arthritis research is small, short, and used oral products rather than flower — and the two best trials found nothing. Topical CBD products are popular for joint pain, but they absorb poorly through the skin and the evidence for them is thinner still. The results justify better trials, not confident promises of relief. Standard arthritis care (anti-inflammatories, physical therapy, exercise) remains the foundation.

Compounds to explore

These are the compounds — terpenes and cannabinoids — that the research connects to this condition. They are starting points for reading a COA or choosing a product, not prescriptions. Each has its own guide with more detail.

Cannabinoid CBD Studied in osteoarthritis trials — the largest, best-designed trials found no benefit over placebo. Terpene beta-caryophyllene Anti-inflammatory terpene with CB2 activity, relevant to joint inflammation. Terpene humulene Anti-inflammatory profile; also found in hops and used in traditional medicine.
