What the research says

Cachexia is not just 'losing your appetite' — it's a metabolically driven wasting syndrome where the body burns muscle and fat even when eating. That distinction explains the disappointing trials. The largest, a phase III study of 243 advanced-cancer patients randomized to cannabis extract, THC, or placebo for six weeks, found no significant differences in appetite or quality of life — 73% of the cannabis-extract arm and 69% of placebo reported improved appetite, and an independent board stopped recruitment for futility.

A 2022 systematic review and meta-analysis (10 studies, 804 patients) reached the same conclusion: no significant benefit of cannabinoids for appetite, weight, or quality of life in cancer cachexia. It is a useful contrast to the 'Appetite Loss' page — THC clearly stimulates appetite in healthy people and in AIDS wasting, but cachexia's metabolic component is not fixed by appetite alone.

The honest caveats

If a cancer patient is losing weight, the priority is oncology and nutrition care — appetite stimulants (like megestrol), tailored nutrition, and treating the cause. Cannabinoids are not recommended for cachexia by current guidelines because the trials failed.
