What the research says
Patient and caregiver reports of CBD helping behavioral symptoms in autism drove a wave of trials — and the better the trial, the weaker the signal. A 2025 systematic review and meta-analysis of randomized controlled trials concluded the current evidence does not support CBD as a treatment for core autism symptoms. The largest recent RCT, published in 2025, gave CBD at 20 mg/kg/day to autistic boys aged 7–14 for a year and found no significant difference from placebo on behavioral or adaptive scales.
Some open-label studies (where families know they're getting the drug) reported improvements in challenging behavior, anxiety, and sleep — which is exactly why placebo-controlled data matters: expectation and reporting bias are strong here. The studies that exist also vary wildly in CBD dose, THC content, and outcome measures.
The honest caveats
Autism affects children, which raises the bar for safety evidence considerably — and that evidence does not exist. THC exposure during development is a particular concern, and CBD interacts with common antiseizure and psychiatric medications. If a clinician and family decide to trial a CBD product for specific symptoms like anxiety or sleep, it should be pharmaceutical-grade, dose-tracked, and reviewed like any medication. No parent should rely on unregulated dispensary products for this.
