What the research says

The entire ADHD evidence base is one small pilot. A 2017 randomized, double-blind, placebo-controlled trial of Sativex (nabiximols, a 1:1 THC/CBD oromucosal spray) in 30 adults with ADHD found no significant difference from placebo on the primary cognitive measure. There was a nominal improvement in hyperactivity and impulsivity symptoms (p = 0.03), and some participants preferred active treatment — but with 30 people, the study was too small to conclude anything.

That is the whole story: no meta-analysis, no replication, no long-term data. The biology cuts both ways — THC can feel focusing at low doses but reliably impairs attention and working memory at higher ones, and heavy adolescent cannabis use is linked to worse attention outcomes.

The honest caveats

ADHD has well-established, well-studied treatments (stimulant and non-stimulant medications, behavioral therapy). Cannabis is not among them — the evidence is a single pilot with a negative primary outcome. At most, a low-THC product might be worth discussing with a psychiatrist as a carefully tracked experiment, with honest expectations that it may help attention, do nothing, or make it worse.
